Payment Options
To keep our fees and expenses down, payment for office visits and/or services is due on the day of your appointment otherwise, a service fee may apply. Payment is accepted in the form of cash or check only.
Please bring your insurance card with you to every visit. You will be asked to sign an insurance authorization form. This allows our billing company to bill your insurance company directly for medically necessary covered services. However you are ultimately responsible for your bill. lf your insurance company does not pay the claim within 90 days, you are responsible for payment of the full balance.
Prescription Refill Requests
Please allow 48 hours for prescription refill requests. This turnaround time is necessary to allow the prescriber to review the request, contact the pharmacy and have them fill the prescription.
